Application to Volunteer

Do I need to do this???

Yes! If you are involved in any one (1) of the following:

* Works in the classroom with students

* Involved in any KS school activity (i.e. dances, socials, etc.)

* Involved in after school activities (i.e. booster club, athletics, Perf. Arts, speech, etc.)

* Room parent, Team parent

* Class parent, ATP parent

* Booster club President or any other position (Vice president, secretary, treasurer)

* Chaperone for any field trip (including during summer school) or overnight camps

* Assistant for athletics, speech, tutoring, mentoring, etc.

* Breakfast aide, Bookfair volunteer, team activities, etc.

Frequently Asked Questions:

Do I need to complete an applicaiton to volunteer EACH year? Yes your application and background check needs to be a "current" application to volunteer every year.

Do I need to complete an application to volunteer for each child? No, just list each child's name and grade on the top right corner of your first application

Can one form (application to volunteer) be used for both my spouse and I? No, it is one application per adult (parent/guardian), so please make a copy before completing the form, or call Hawley Cappo at 842-8659 for another form.

I don't want to release my SSN# due to identification theft. We understand your concern. SSN's due are necessary to conduct the yearly background check and therefore are NOT listed on our volunteer system.

Last year I took a TB test, do I need another one this year? No as long as we have your current TB test results on file. If you are not sure, call Hawley CAppo at 842-8659.

I just had a TB test done form my job; di I need another one for Kamehameha? No, if your test results were done within the past 12 months, just make a copy of your certificate/card and submit with your application.

I tested positive for TB and had a chest x-ray done, do I take another test?Yes, every 6 years a new test should be done.

Should I mail in my TB card or a copy of the card? A copy of your TB card is sufficient. If you mail in your cared, we will return your card to you in the mail.

I am a KS employee do I need to complete an application to volunteer? Yes, so that your name can be attached to your child. Please notate next to your name (KS employee).
